Referrals

Helping You Find the Right Care

At Dukeries Healthcare, we work closely with families, local authorities, and healthcare professionals to support smooth and timely care placements.

Who Can Make a Referral?

We welcome referrals from:

Social Workers

Local Authorities

Hospitals and Discharge Teams

Family Members and Carers

Individuals seeking Care for Themselves

If you are unsure whether one of our homes is suitable, our team will be happy to discuss the individual’s needs and provide guidance.

What Happens Next?

Once we receive a referral, our team will review the information and arrange an assessment if appropriate.

Our process usually includes:

  • Reviewing the referral information
  • Discussing care needs with the referrer or family
  • Completing a pre-admission assessment
  • Confirming whether the home is suitable
  • Arranging admission where appropriate

Our priority is always to ensure the placement is safe, appropriate and supportive for the individual.
